This form is to be completed if a student requests a schedule change. The student should complete the form and obtain a parent/guardian electronic signature before submitting the request. The student should follow his/her original schedule until his/her schedule has changed in Infinite Campus. All schedule change requests must be submitted through this form. Last day for submitting a schedule change is August 11, 2022 by 3:30 p.m.
