School-Based Flu Clinic: DEADLINE EXTENDED - forms are due back on Monday, September 28

The Thomas County School System is happy to partner with the Thomas County Health Department in offering flu vaccines to our students at no cost this fall.  An information packet was sent home with Garrison-Pilcher, Cross Creek, Thomas County Middle, and Thomas County Central High School students regarding the vaccine.  If you did not receive a packet, you may download the information below or pick up a packet from your child's school. If you would like for your Garrison-Pilcher, Cross Creek, Thomas County Middle, or Thomas County Central High School student to receive the flu vaccine, please complete and return the HIPPA acknowledgement and vaccine consent forms by Monday, September 28.

Your child will not be able to receive the vaccination if the completed forms are not returned to the school by the due date. Both the injectable and intranasal forms of the vaccination will be available. If your child has health insurance, please provide the insurance information on the consent form, and your insurance will be billed for hte vaccine. If you do not have insurance or if your insurance does not cover the cost of the flu vaccine, the vaccine will be provided to your child at no cost.

Flu vaccines will be administered at Hand-in-Hand on September 24, Bishop Hall, Pathways, and The Renaissance Center on September 28, Thomas County Central on October 13, Garrison-Pilcher on October 19, Cross Creek on October 21, and Thomas County Middle on October 22. If you have any questions about the flu vaccine, please contact the Thomas County Health Department at 226-4241.
