Copy of up to date Immunization Record (available at Thomas County Public Health Department)
PPD (TB) screen and a 5 drug screen panel- May both be obtained from Archbold Urgent Care for about $40 total. Please call 228-4136 to schedule an appointment.
ALL OF THE ABOVE IS DUE THE FIRST WEEK OF SCHOOL. I MUST HAVE DOCUMENTATION IN YOUR FILE TO BE IN THIS CLASS. I MUST SEE YOUR ID CARD AND SOCIAL SECURITY CARD, I CAN NOT ACCEPT A COPY.
Uniform- $35-will be ordered
Solid white non-slip sole shoe- $20-30 may not be canvas, must be full shoe, no CROCs, no holes or cut outs. Check at Wal-Mart. Needed by December.
Liability Insurance- $15 -will be purchased thru TCCHS
Background check- Thomas County Sheriff's Department- $10- application will be completed at school
CNA certification test-$112 The test will be paid for by the school ONE time. Our pass rate for the past 3 years has been 93% or above. Students that study and are in school daily should pass the state test on the first try. Students that do not pass on the first try will be scheduled to retest and the parent will be responsible for the fee. (Hint Hint: be in school and study!)
