New Employee & Open Enrollment Forms

Personnel Information - For name changes submit this form with a copy of your social security card with your new name.

Change of Address Form - This form should be used for changing the mailing address on your paycheck.   Submit this form to Payroll.

School Experience Verification Form

Direct Deposit Enrollment - Submit this form to have your paycheck directly deposited into your checking account.

Form G-4 - Employee's Withholding Allowance for Georgia State Income Tax.

Leave Forms

Maternity Leave - You need to complete four separate forms: Maternity Request Form 1, Maternity Physician's Statement Form 2, Maternity Physician's Confirmation of Pregnancy Form 3, and Physician's Statement Following Pregnancy Form 4. Return the forms to Miranda McInnis as soon as they are completed.

The Family and Medical Leave forms must be completed if you miss an extended amount of work (one week or more) due to the care of a new child, care of a family member, an employee disability and/or military leave. FMLA has many restrictions. Contact Miranda McInnis for more information. If you qualify, you must complete the Request Form and the Physician's Confirmation of Employee Disability Form


Sick Leave Bank

The Sick Leave Bank (SLB) has been established to provide additional sick leave to members whose personal sick leave and other available leave has been exhausted and who are unable to perform their assigned duties for an extended period of time due to their own catastrophic illness or the catastrophic illness of thier child, spouse, or parent. Employees must be a member of the SLB to be eligible to particiapte.  Below you will find the Enrollment/Withdrawal application (for use during Open Enrollment periods), the Request form (for use when requesting days from the Sick Bank), and the SLB Guidelines.

Separation of Employment

The Separation of Employment Form must be completed if you are leaving the employment of the system.  This will be mailed in your seperation package.

The Exit Interview will allow the system to obtain valuable information from those leaving our employment.
